Claude doesn't say "I'm not sure." It says "here's your pipeline summary." And it's wrong in a way that looks exactly like being right.

Revenue agents are reporting the wrong pipeline number and teams have no idea. We connected HubSpot, Gong, Stripe, and Slack via MCP and asked for a pipeline summary. Claude reported $1.2M. The actual number was $740K. No uncertainty flagged, no caveats. A confident answer built on an incomplete foundation.

Revenue agents are ranking the wrong rep and the output looks clean. We asked Claude to rank rep performance. It identified the wrong person at the top because closed-lost reasons hadn't been encoded. Nothing in the response suggested a problem.

Revenue agents can't see the accounts about to churn. We asked Claude to flag churn risk across the account base. It marked three accounts as healthy. Two churned within 30 days. The signals existed in the data. The context needed to surface them didn't.
